Family-Managed Home Care (FMHC)
What is FMHC?
Family-Managed Home Care (FMHC) is a program from the Ontario Ministry of Health that gives families more choice and control over home care. Instead of home care services being arranged for you, families receive funding directly. This funding can be used to hire care providers or purchase services that fit the client’s care plan.
How to Apply
To apply for FMHC, families first connect with Home and Community Care Support Services (or CHEO for children). A Care Coordinator will:
Assess the client’s needs
Create a care plan
Check if the client and family are eligible for the program
Who Can Qualify?
The FMHC program is available to specific groups:
Children with complex medical needs
Adults living with brain injuries
Eligible home-schooled children
Clients with special or extraordinary care needs
Families also need to show they can manage responsibilities like hiring, scheduling, and supervising care providers, as well as reporting how funds are used.
